Additional appointment times and fees will be based upon individual need.
If you have not been seen in our office for the last nine months, it is absolutely essential that your follow up appointment is scheduled for 60 minutes so that we can address all of your concerns to give appropriate care. If your appointment only takes 30 minutes, you will only be charged for the time spent.
Policies:
* Payment is expected at time of service.
* There will be a full visit charge for cancellations made within 24 hours of appointment time.
* In order to allow us to provide more comprehensive visits, we do not participate with insurance. We will provide you with the paperwork you will need to submit for reimbursement.

**At your first appointment it is best to bring in any vitamins, supplements, and/or medications you are currently taking. You will be meeting with Dr. Stewart for a medical evaluation and consultation.

Information for Patients on Thyroid Medication
Do not take any food within 1-2 hours of taking your prescription thyroid medication. If taking supplements, wait a minimum of two hours and preferably four; however, if you are on a supplement three or four times a day, you may only be able to wait two hours. The most important thing is to be consistent in the time you take your medication and the time you eat and take supplements. Calcium and iron supplement must be taken at least four hours away from thyroid replacement medication. When having your blood levels checked, fast that morning from everything but water and do not take your thyroid medication until AFTER the blood is drawn. We have "opted out" of Medicare participation, and we have privately contracted with you to provide care and services. Medicare will not accept bills submitted by patients and will not pay for any services ordered by us. Please understand that you are responsible for all services rendered by us.
